Bayview Family Practice
We are a four physician office located in midtown Toronto. We serve 6000 patients and provide after hours care as part of a Family Health Organization. Our vision is to provide enhanced primary health care services to patients while supporting our health care providers and staff. Bayview Family Practice is associated with the Department of Family and Community Medicine at the University of Toronto and our physicians hold privileges at Sunnybrook and Women's College Hospitals.
The Otter Creek Family Health Organization
The doctors of Bayview Family Practice are part of the Otter Creek Family Health Organization (FHO). This enables enhanced continuity of care for our registered patients beyond regular hours through After Hours Clinics provided in collaboration between all physicians in the FHO.

As our clinic is affiliated with the University of Toronto Faculty of Medicine, you may be attended by a medical student or resident prior to seeing your regular doctor.
The clinic utilizes Oscar Electronic Medical Records for completely paperless patient records and uses Doctors Services Groups to administer our uninsured services.
