Important message from your doctor

Dear Patients,

For those continuing their care with me at the Women’s College Hospital Family Practice Health Centre (FPHC), I want to provide you with more details to ensure the transition is as seamless as possible for you.

When: You may now call and schedule both in-person and virtual appointments for January 2, 2024, and onwards.

How: Please call 416-323-6060 press 3 then 1.

Where: 77 Grenville St. across from the main hospital site. 3rd Floor Red Team

Parking: There is metered parking on the adjacent streets as well as in the hospital parking garage.

TTC: FPHC is easily accessible by transit, located a short walk from both College St. and Queens Park Subway Stations.

Plan: Please allow yourself extra time for the first visit to orient yourself to the new location. Upon arrival, check in with the Red Team Secretary to verify your information. There is also a check-in kiosk for your convenience.

We continue to be available to you at Bayview Family Practice for urgent medical concerns and after-hours care until December 31, 2023.
